According to Spielvogel, "acts of exhibitionism, voyeurism, autoeroticism, and oral intercourse abound, however, due to the 'morality' of the patient, neither the fantasy nor the act leads to genuine sexual satisfaction, but rather to predominant feelings of shame and fear of punishment, especially in the form of castration." Spielvogel believes that many of the symptoms can be traced to the bonds that dominate the mother-child relationship.
